Being well-prepared for your appointment will ensure that the doctor has all necessary information to provide you the best possible care. It also will help relieve any unnecessary anxiety you may be feeling.Educate yourself on your symptoms by reviewing our Patient Education section. Also, take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.

To expedite the process and reduce waiting time, we encourage you to fill out our Demographic and Insurance Information Form.

We also encourage you to complete and sign a Release of Information form if you would like us to obtain your medical records from other physicians or send your records to them upon your request.

Appointments

We encourage you to make your future appointments on the day of the visit before leaving the office. An appointment card will be provided, and you will receive a reminder one or two days before your scheduled visit, but it remains your responsibility as well.

Please remember to bring your insurance card if there is any change in your insurance. It is also recommended to bring your medications or a list of your current medications. Please inform the receptionist about any change in your address, telephone number, or insurance carrier to keep your records updated.

Cancellations

If you cannot keep your scheduled appointment, please call us at least 24 hours in advance.

For canceling the stress Cardiolite test, it is required that you call us at least one day in advance. Otherwise, you will be responsible for the cost of Cardiolite and other related medications such as Lexiscan and Adenosine, as your insurance company will not cover these expenses.

Prescription Refills

It is the policy of CVI to refill prescriptions that have been originally prescribed by CVI doctors during weekdays from 8 AM to 5 PM. Prescriptions cannot be completed during non-business hours because your medical records are not available, which could lead to errors. Please allow at least 3 to 5 days for your refill request to be processed.

Please call CVI only for prescriptions provided by CVI doctors. If you have not seen a CVI doctor for an extended period, you may be asked to visit the office before the medication can be refilled. Certain cardiac medications require close monitoring, including blood tests and EKGs, to ensure safe and effective treatment.
